Schedule an appointment or contact counselor to receive password for counseling forms.  Before we get started with counseling, please download and complete the following forms.  This document is password protected.  Please use the password provided by counselor to open the PDF.  Within in the information packet is a Client Information sheet, a Consent Form, and an Initial Intake.  Please sign, complete, and return before our first session together.  If you have any questions or concerns, please don't hesitate to write.  I look forward to hearing from you.
